What's included
A year of preventive care — plus a discount on everything else
Cleanings
Two routine cleanings per year — or two periodontal maintenance cleanings, whichever your care plan calls for. Additional cleanings are 20% off member pricing.
Exams & X-rays
One new-patient or recall exam with cleaning. One problem-focused exam. One set of bitewing X-rays (includes front-view X-rays for adults).
20% off most treatment
Fillings, crowns, bridges, periodontal treatment, dentures and partials, non-molar root canals, extractions, custom night guards, fluoride, and sealants.

Family enrollment
Sign up as a household — pay for one person per month
If multiple family members are seen on the same day and want to enroll, you can pay for one person monthly starting that day. Your 12-month term starts that same day for everyone in the family.

The details
- This is a dental savings plan — not insurance. It's a direct agreement between you and our practice.
- Membership is good for 12 months from the date of purchase.
- Valid only at Birdwell & Mutlak Dentistry. Cannot be combined with insurance or other discounts.
- The membership fee is non-refundable.
- Excludes veneers, cosmetic procedures, and in-office retail products.
